merge two column into single columns and show them in separate rows

My Table looks like this

Name    Date1   Date2   Date3
A   25  23  20
B   20  15  10

I want it like this

Name    Date
A   25
A   23
A   20
B   20
B   15
B   10

how i can achieve this result using sql query
